The Check, Think and Act is a colour, A4 sized paper resource designed by Jayne Lehmann to make it faster, easier and more effective to help people to understand what their blood glucose levels mean and the actions that can be taken to keep the levels in the healthy range for longer.

Once purchased, an email will be sent to you to click on the hyperlinked name of the resource to download it from this website. Easy to follow instructions are included to show you how to use it as both a teaching resource (once laminated) and take home patient handout.

It is designed to simplify the process by using the number on the meter before the decimal place. This is matched to one of two number lines (before or 2 hours after a meal, to see if it is low (red), in the healthy range (green) or high (orange). Underneath the colour sections are actions the person can take to get their numbers back into the healthy range. The handout reminds people to check the number, think about what it means and then act on it for good diabetes health and wellbeing.
