Jayne Lehmann is a South Australian Credentialled Diabetes Educator Registered Nurse helping grassroots health professionals to strengthen their diabetes care and education skills.

No matter what your health profession, we all see people with diabetes. Around fifty percent of the one million Australians living with diabetes have high blood glucose levels which increase the risk of poor health. A lot can be achieved when the health team around a person with diabetes delivers and reinforces the same contemporary diabetes care, which means keeping your diabetes care knowledge and skills be up-to-date.

 Who are grassroots health professionals?

Any health professional providing health services to people with diabetes! This includes Aboriginal health workers, Credentialled Diabetes Educators, diabetes educators, dietitians, exercise physiologists, GPs, nurses, occupational therapists, opticians, paramedics, pharmacists, physiotherapists, podiatrists, psychologists and disability or aged care support workers.

How do health professionals get involved?

1. Sign-up for diabetes coalface newsletters and other communications filled with practical and evidence based articles on all things diabetes. You can subscribe at the bottom of this page and check out past editions here: newletter page.

2. Explore the information and resources on this site  Drop-in and check what’s been added! There’s information, resources, tips and activities to strengthen the delivery of evidence based diabetes care and education  Diabetes education resources . Download the resources to use in your clinical practice.
