Report

EdHealth’s diabetes educators can write reports for NDIS participants with diabetes. After an assessment, the report is written to inform others of the support a NDIS participant and their circle of support will require, for the NDIS participant’s diabetes care to be put into action, like they would if they did not have their disability.
NDIS participants and their support coordinators/family engage us to provide reports for a number of different situations.
This includes:
- Support for a ‘Change of Circumstance’; application
- Support for the review a NDIS Plan funding decision
- Informing NDIS planners of diabetes supports we believe are required for the participant to be supported in their diabetes care
- Summarise and provide feedback on services EdHealth has provided to improve the capacity of a participant in their diabetes care and evaluate its impact. The future needs of the participant are also provided.
Report inclusions
The reports are comprehensive and include:
- Summary of disability impact on capacity to do their diabetes self-care
- Summary of diabetes care needs and support requirements
- Recommendations of specific support strategies to be used with the client for capacity building for their active involvement in their diabetes care
- Recommendations for self-care routines the NDIS participant and their circle of support are to do
- Identification of missing or inadequate diabetes healthcare by the NDIS participant’s diabetes team and recommendations on how to correct this within local health services
- How to maximise the impact of their Diabetes Health Team.
The EdHealth services recommended in the report focus on the disability side of the participant’s diabetes care, while also providing a review to identify how they can get the healthcare for their diabetes they are entitled to within our Australian and state funded health services. At the end of the report is a quote for the EdHealth Diabetes 4 Disability Services identified as being able to address the barriers identified in the report. Quotes are valid for 2 months.
