EdHealth Australia is a Registered NDIS Provider of Diabetes 4 Disability (D4D) support solutions across Australia. We employ Registered Nurse Credentialled Diabetes Educators with an advanced understanding of disability support. Combining health and disability understanding magnifies our impact by creating effective systems of support around each person with diabetes. This includes support worker training on diabetes self-care and the equipment the person is using to manage their diabetes. A diabetes management plan to guide support workers with person-centred support and the active inclusion of the person with diabetes in their self-care. Plus picture resources can support the individual to be more actively involved!
We focus on each person with fresh eyes, and the wisdom of experience doing this work over many years.

EdHealth began when Jayne Lehmann, a Registered Nurse and Credentialled Diabetes Educator, started her work as a sole practitioner in 1996.

EdHealth’s staff all have lived experience of disability. Their deeper understanding of disability grounds their work with people with disability and their support team at a deeper level of mutual understanding. We know everybody’s situation is different and ask better questions to find out how we are best able to support the person with a disability and the different members of their team involved in providing their diabetes support. We want to improve the lives of people with a disability by addressing the impact of their disability on their capacity to self-care like other people with diabetes. We celebrate their successes and work hard to find solutions to their challenges and keep them on track to good health, despite their disability!
