Description
The Check, Think and Act, is one of a suite of resources within the Diabetes 4 disability (D4D) Capability Framework, increasing health equity for people with diabetes doing glucose monitoring.
EdHealth’s 2026 version of the Check, Think and Act (CTA) is a diabetes education resource and handout supporting people to understand their glucose level and actions to take to keep their glucose levels in range longer. More time in range = better health outcomes for people with diabetes.
Using simple number lines, 4 colour-coded glucose ranges and clear action prompts, CTA reduces the health-literacy and numeracy demands involved in interpreting glucose results.
The pack includes five CTA versions with ideal and higher targets for people with type 1 and type 2 diabetes. Each is further customisable for other targets, allowing health professionals to select a CTA to match the person’s treatment, hypoglycaemia risk, clinical recommendations and glucose targets.
CTA teaches a simple three-step approach:
CHECK the glucose level
THINK about what it means
ACT when action required
A key feature is the use of the whole number before the decimal point to make glucose results easy to match on the visual number line. This does not replace the actual glucose reading for clinical or treatment decisions; it is an education strategy designed to simplify interpretation and support self-management.
The pack includes:
- five colour CTA resources covering different diabetes treatment and risk profiles;
- guidance to help health professionals choose the appropriate version;
- instructions for preparing and using the resources;
- practical guidance for teaching CHECK–THINK–ACT;
- information for using CTA at home; and
- a limited licence for up to 5 people employed within the same organisation, to use in their clinical practice
Ideal for Credentialled Diabetes Educators, nurses, GPs, pharmacists, dietitians, exercise physiologists and other health professionals looking for a practical, visual way to support people to proactively use their glucose levels for good health.
From checking a number … to understanding what it means … and knowing what to do next. The Check, Think and Act supports people to achieve their glucose targets and spend more time in range!
