Providing the support workers with a presentation on the use of the Diabetes Management Plan (DMP) is recommended when an EdHealth DMP is prepared for a NDIS participant. This service will improve how the DMP is used by their support workers and complements the training provided in the D4D online training.

A PowerPoint presentation is created to reflect the NDIS participant’s DMP by one of EdHealth’s Credentialled Diabetes Educators/diabetes educators. This is presented by them via the Zoom online session, which is booked for a mutually convenient time. Interaction is encouraged as the specific strategies for the NDIS participant are introduced and discussed.

This opportunity for deeper discussion with the team delivers a number of advantages, including:

  • Team discussion about the implementation of the information
  • Identification of any issues requiring greater consideration or clarity

This strategy sets the support workers up to successfully and consistently support their client with their diabetes care. It will also further develop the team’s approach to providing the support outlined in the DMP.
